24(S)-Hydroxycholesterol is a brain-derived oxysterol and a potent endogenous agonist of liver X receptors (LXRs), which regulate cholesterol homeostasis and inflammatory gene expression in the central nervous system. As a major cholesterol metabolite in the brain, it plays a critical role in maintaining neuronal lipid balance. Elevated levels of 24(S)-hydroxycholesterol in cerebrospinal fluid have been associated with early-stage Alzheimer’s disease and mild cognitive impairment, making it a valuable biomarker for neurodegeneration. Additionally, its neurotoxic effects on neuronal cells underscore its dual role as both a physiological regulator and a potential contributor to disease pathology.
