It serves as a substrate for aminoglycoside acetyltransferase and aminoglycoside phosphotransferase, yet it is not modified by various common aminoglycoside-modifying enzymes, and it selectively inhibits MATE2-K. It is active against multidrug-resistant bacteria, including carbapenem-resistant Enterobacterales[1][2]. It has the molecular formula C25H48N6O10 and a molecular weight of 592.68 g/mol.

In Vitro

Against Klebsiella, Escherichia, Enterobacter, Serratia, and Citrobacter species, Plazomicin shows MIC50/90 = 0.25-0.5 μg/mL/0.5-1 μg/mL[1]. Plazomicin selectively inhibits MATE2-K, and to a lesser degree MATE1 and OCT2, which are important transporters in tubular secretion[1]. Plazomicin (18 h) keeps its antibacterial activity against most E. coli BW25113 and E. coli BW25113 ΔbamB ΔtolC strains that express aminoglycoside modifying enzymes[2].
